چکیده

A review of world literature on ajamline has been attempted; almost two third of the work is available in the German language. Ajmaline was isolated from rauwolfia serpentina, by Dr. Salimuzzaman Siddilui, and named after Hakim Ajmal Khan who who has used products of rauwolfia as antihypertentive and cardiac sedative. Antiarrhythmic effects of ajmaline are due to depression of atrioventricular conduction with prolongation of P-Q, QRS and Q-T intervals. It increases the refractory period of the accessory pathway and lengthens the H-V interval in Wolff-ParkinsonsWhite syndrome and thus prevents associated tachycardias. Despite its origin from the Indian subcontinent, the drug is not in vogue in this part of the world.

Unusual response to the ajmaline test in a patient with Brugada syndrome.

We present a Brugada syndrome patient who suffered an aborted sudden death. The ajmaline test (1 mg/kg body weight) induced accentuated alternans ST-segment elevation in V1-V2 without ventricular arrhythmias. It could represent silent ischaemia not detected before, failure of myocardial regions to repolarize in alternate beats due to transmural dispersion of conduction and refractoriness in the...

Malignant ventricular arrhythmias induction by programmed electrical stimulation of the right ventricular outflow tract only during type 1 Brugada ECG maximization.

OBJECTIVE The role of electrophysiology study in Brugada syndrome (BS) sudden cardiac death risk stratification remains controversial and seems to depend on the phenotypic expression of the channelopathy. Ajmaline has a key role in the diagnosis of BS. We observed that programmed electrical stimulation (PES) of the right ventricular outflow tract (RVOT), only when type 1 BS ECG is unmasked by a...

Emergency diagnosis and management of common pediatric tachyarrhythmia

Tachycardia is common in the pediatric age group. The most commonly seen arrhythmias in children are sinus tachycardia. However, supraventricular tachycardia is the most common tachyarrhythmia that necessitates treatment. Ventricular tachycardia is rare in healthy children but may be associated with sudden cardiac death.  The important risk factors that predispose children for arrhythmias inclu...

Correlation of intracardiac electrogram with surface electrocardiogram in Brugada syndrome patients.

AIMS The objective of this study was to correlate the electrocardiogram (ECG) modification during an Ajmaline challenge in patients affected by the Brugada syndrome and implanted with an implantable cardioverter-defibrillator (ICD) with the morphological changes of their ICD's intracardiac electrogram (IEGM). METHODS AND RESULTS Sixteen type 1 Brugada syndrome patients implanted with a St Jud...
